Asthma in children is one of the major causes of emergency department visits. The assessment of the asthma severity and a stepwise approach to pharmacologic treatment can lead to an improved short-term outcome of the asthmatic children. After require prompt treatment, some will go home, others will require hospitalization. Clinical practice guidelines help to optimize decision support to emergencies. The effectiveness of these protocols depends on the assessment of the severity of the crisis, the quality of the protocol and its implementation in practice. In this article, we present a clinical practice guideline bases on updated evidence and on the experience of a multidisciplinary team. The treatment of exacerbations has been subject of many recent publications. The β2-agonists by inhalation are in all cases indispensable. When indicated, oral corticosteroids should be given early.
