New algorithms based on trough levels of anti-TNF and their antibodies are suggested in patients with inflammatory bowel diseases presenting loss of response under treatment. Conversely, monitoring of trough levels and optimization of treatment without clinical relapse have not proved its usefulness in clinical practice. In case of de-escalation, trough levels could be interesting before to switch to new treatment. The cost-efficacy analysis of trough levels measurements has been reported interesting.
