New endoscopic tools, such as wireless capsule endoscopy and improvement of cross-sectional imaging have been associated with an increased number of patients with ileal lesions, discovered sometimes without any symptoms. Even if Crohn's disease is the main cause of acute ileitis in western countries, many other causes are possible, mostly infections. Aims of the present review were, firstly to propose a systematic approach to manage patients according to their clinical presentation by a step-by-step diagnostic strategy and second, to discuss the differential diagnosis of an ileal Crohn's disease.
