Crohn's disease is considered to be the main cause of acute ileitis in western countries. In current practice, other diagnosis should be discussed and explored, mostly infections. Differentiating intestinal tuberculosis from Crohn's disease remains a diagnostic challenge. New endoscopic device and the improvement of radiologic technique increased the number of patients with ileal lesions without any symptoms. Except in patients with spondyloarthritis, little is known about the natural evolution of these features. The significance of ileal lesions detected with the wireless capsule endoscopy should be investigated. The current review will propose diagnostic guidelines according to the patient's clinical presentation.
