Postural hypotension is defined as a fall in blood pressure of over 20 mmHg systolic or 10 mmHg diastolic on head-up postural change. It may be a presenting feature in primary or in secondary neurogenic failure. The therapeutic strategie includes both non-pharmacological and pharmacological measures. The goal of treatment is to obtain a target blood pressure and an improvement of patient's functional capacity. A large selection of pharmacological agents allows to stabilize the disease process in autonomic disorders such as midodrine, fludrocortisone... The management of orthostatic hypotension is dependent upon an individual approach.
