Posterolateral approach for the treatment of trimalleolar ankle fractures (a prospective study of 22 cases) Volume 27, issue 1, Mars 2011

The treatment of trimalleolar ankle fracture requires particular attention to the posterior marginal fragment. The purpose of this paper is to study the effect of the posterolateral approach for this type of fracture. We performed a prospective study of 22 cases over 2 years. The average age of the patients was 39 years; men were affected in 80% of the cases. Traffic accidents were found to be the principal cause of the fracture. We noted a predominance of intertubercular type fracture. For the posterolateral approach, the patient was positioned in posterior or lateral position. This approach permits good exposition, reduction and osteosynthesis of the lateral malleolus and later for the posterior marginal fragment. According to the Vidal ankle score, clinical and radiological results were good in 72.7 %, acceptable in 18.1 % and bad in 8.2% of cases.
