Background: The number of patients treated with oral chemotherapy has dramatically increased in the past few years. This suggests a transfer of responsibilities from hospital towards community pharmacies and patients. Purpose: To report an experience of clinical pharmacy services with cancer patients treated with oral chemotherapy. Methods: At day hospital, in our cancer care institute, a multidisciplinary program called METEOR to assist cancer patient was created. The Pharmacists take part in this multidisciplinary approach to carry out patient empowerment by delivering personalized information on their treatment. They also provide comprehensive medication review and analyze the risk of drug-drug interactions. Results: 130 pharmaceutical consultations were conducted between February 2014 and December 2015; 14 different drugs were considered. Conclusions: This multidisciplinary approach contributes to optimize safe use of drugs. The pharmacists, by providing medication information to the patients contribute to the improvement of patient adherence and seamless care. An evaluation is warranted to further demonstrate these issues.
