Cytomegalovirus infection remains the most deleterious viral infection after kidney transplantation. However, a strategy of prevention is now available, either chimioprophylaxis or preemptive therapy guided on virologic monitoring. Both attitudes seem to be equivalent with regard to efficacy. The questions of cost-efficacy and emergence of antiviral-resistant-CMV strains remain to be better evaluated.
