Answers

(1) Interictal SPECT has a poor localizing value. Ictal SPECT has a much better localizing significance.

(2) Early injection and image fusion of ictal and interictal subtraction SPECT on coregistered MRI (SISCOM).

(3) As shown for our patient, SISCOM is capable of demonstrating two different seizure onset zones reflecting different semiology (including subclinical features) which are associated with different EEG seizure patterns. Without ictal EEG, we would not have been able to adequately interpret our SPECT findings.
