An 80-year-old Japanese man presented with a fist-sized indurated reddish tumour on the left axilla that had appeared two years previously (figure 1A). Skin biopsy showed epithelioid cell granulomas in the deep dermis and subcutaneous tissue (figure 1B). Mycobacteria were detected in the culture of skin tissue and pus, and mycobacterial avium complex (MAC) was identified using polymerase chain reaction (PCR).
