A 65-year-old man was admitted to the hospital with the presentation of fever and pharyngalgia. Initial hematological results showed leukocyte of 232 × 109/L, hemoglobin (Hb) concentration of 12 g/dL, and platelet count of 38 × 109/L. Peripheral blood smear revealed 98% blast cells with medium size, 10% blast cells with prominent nuclear invaginations, identified as cup-like blasts.
