A 76-year-old woman presented with ulceration of the palm of the hand that appeared five months ago. She had received methotrexate (12.5 mg/week) and prednisone (5 mg/day) for rheumatoid arthritis for six years. Physical examination revealed a 7.5-cm-wide sharp-contoured and painless ulceration of the palm, without fibrin, necrosis or sensitive disorder (figure 1A). General examination was otherwise normal.
