A 76-year-old Japanese male manifested with bullae and erosions for two months. His medical history included diabetes mellitus and schizophrenia. He had taken vildagliptin, a dipeptidyl peptidase-4 inhibitor, for seven months before the onset of the skin lesions. Physical examination revealed multiple flaccid blisters with a clear and bloody content and erosions with erythema on the entire body (figure 1A).
