A 48-year-old man presented with a two-month history of generalized pruritic erythematous plaques and nodules (figure 1A, B). His skin lesions were mainly distributed over the face, trunk and arms, sparing his palms, soles and mucosal surfaces. Palpable cervical lymphadenopathy was noted. A skin biopsy showed granulomatous inflammation with a mixed infiltration consisting of numerous plasma cells, lymphocytes, and neutrophils throughout the dermis, suggesting a spirochete infection (figure 1C, D).
