A 20-month-old boy presented to our dermatology clinic due to a sudden-onset disseminated papulosquamous dermatosis associated with perifollicular keratotic papules and “islands” of spared skin (figure 1A, B), involving the scalp, face, trunk, anogenital area, elbows, and knees. In addition, he presented with waxy palmoplantar keratoderma (figure 1C) and onychodystrophy. Discrete pruritus was evident.
