A 48-year-old Japanese man noticed painful left submandibular and cervical lymphadenopathy 12 days before admission to our hospital. Within a few days, he developed general malaise with fever, bilateral cervical lymph node swelling, arthralgia, erythema on the extremities, and swelling of the hands and feet. Antibiotic treatment was unsuccessful. On admission, he had hyperaemic conjunctivae, weak erythema of the lips with scales/crusts, perleche, and strawberry tongue.
