A 85-year-old woman presented to the emergency department with a 2-week history of fatigue and dyspnea. Abdominal scan revealed a mediastinal mass and a pleural effusion.Subsequently, pleural fluid cytology revealed an extensive infiltration by large cells with basophilic and vacuolated cytoplasm, suggesting a high-grade lymphoma (figure 1A).
