Prevention of gastric cancer by helicobacter pylori eradication: Who to select? Who to monitor? Volume 20, supplement 1, Janvier 2013

Helicobacter pylori may cause gastric cancer. The increase in relative risk related to the infection is 30-40 fold. Seventy five percent of cancers are related to Helicobacter pylori. Early eradication is especially efficient. However, it remains beneficial in patients with preneoplastic lesions (atrophy, intestinal metaplasia). In countries with a low incidence of gastric cancer, including France, high risk patients should be selected in order to be searched for Helicobacter pylori and to be treated: first-degree relative of a family member with a gastric cancer, patients with preneoplastic lesions, patient originating from countries with a high incidence of gastric cancer, patients with HNPCC, patients receiving long-term treatment with PPI. Helicobacter pylori should be searched for with gastroscopy associated with biopsies in patients older than 45 years and with urea breath test in patients younger than 45 years in the absence of symptoms.
