Essential anorectal pain syndrome (normal clinical examination with non-specific imaging) is relatively frequent in general population. Physiopathological mechanisms are poorly known (except for pudendal neuralgia) and medical literature is heartbreaking. The therapeutic strategy is more or less the same for each condition with drug treatment in the first instance and infiltrations with local anesthetics in case of failure, despite poor results. Surgery can be mandated in pudendal neuralgia (and sometimes coccygodynia) in highly-selectioned patients. Physical therapy could also be effective in most of these syndromes (except in pudendal neuralgia) although it is poorly studied.
