Sulfamethoxazole (SMX) is known to cause crystalluria. Several cases of renal impairment associated with precipitation of the SMX metabolite as crystals of N-acetylsulfamethoxazole hydrochloride (NASM) have been reported in the literature. Risk factors associated with the development of NASM crystals include high levels of SMX, low daily urine volume, urinary pH < 5.7, hypovolemia or dehydration, pre-existing renal insufficiency, and hypoalbuminemia [1].
