Résumé : The relationships between high serum levels of cholesterol and cardiovascular diseases have now been established beyond any doubt. Five recent major trials carried out both in primary and secondary prevention showed that the incidence of cardiovascular event decreased by 30 % in the group treated with statin compared to the reference group given a placebo. These studies showed that the benefit in terms of number of patients to treat to avoid one event depends on the global risk of the target population. This number is an indication of the cost effectiveness of the intervention in a given group of patients. These results lead to new recommendations for treating hyperlipidemic patients : four threshold levels of LDL-cholesterol (220, 190, 160 and 130 mg/dl) were selected to indicate to the treating physician when a lipid lowering drug should be given. These levels depend on the number of cardiovascular risk factors. This paper discusses how the data of these major trials may or may not be extrapolated to other populations. |
