In 2018, pulmonary arterial hypertension has undergone major changes as objectives of treatment, interest of therapeutic associations at diagnosis (whose evidence is mounting), non-pharmacologic treatments and new metabolic pathways, especially epigenetic, targeted with molecules under development working on right ventricle, pulmonary circulation and skeletal muscles, supporting the concept of a systemic disease with pulmonary vascular starting point. The irruption of the microRNAs as biomarkers and therapeutic targets represents an actual hope for these patients.
