Venous compression is part of the therapeutic arsenal for the treatment of proximal deep vein thrombosis. The main studies showed an interest in the use of compression socks of 30-40 mmHg set up at diagnosis and worn regularly due to the improvement of initial symptoms and prevention of post-thrombotic syndrome. The SOX study questioned the value of compression. The large heterogeneity of the randomized studies and the results of the meta-analyses are at the origin of the absence of a current recommendation. However, the latest studies (OCTAVIA, IDEAL-TVP) show the effectiveness of compression in this indication, subject to good compliance and probably a “patient-dependent” adapted prescription period on the Villalta score of three and six months.

A reassessment of the place of compression is necessary through well-conducted double-blind therapeutic trials, with good observance and taking into account the cost-effectiveness factor.
