The hepatic hydatid cyst is a disease caused by the ingestion of Echinococcus granulosus, quite common in the Maghreb countries. Its mechanical invasive character on the bile ducts is classically known, however its spin-off and latent development in the venous portal system is exceptional. Portal hypertension with portal cavernoma is not much described in the literature. Our observation is unprecedented and documents the portal venous localization of a KHF and shows the development of collateral carriage port-auxiliary door secondary to this barrier portal. Echodoppler and triple phase scanner help the diagnosis; lesion mapping is fairly well documented. Finally, the appearance of a portal cavernoma following a break-in and swarming of a KHF in the portal venous system remains an exceptional complication described in the international literature.
