The bleeding and the transfusional needs during the perioperative period are major concerns. The platelets are often implied in these bleedings but their transfusional compensation is not always easy to identify. Starting from explanatory data of the normal platelet function and its physiopathology after a CEC, proposals for biological explorations are made as well as proposals with therapeutic implications. Algorithmic approaches are proposed in order to improve the transfusional efficiency.
