Multiple arterial dissections involving the carotids and the aorta are unusual situations, rarely reported in the literature. They constitute a cardiovascular emergency with poor prognosis and high risk of mortality. We report the case of a 44-year-old man, with a history of chronic alcoholism and smoking and hypertension, who presented to the emergency room with acute dyspnea and stroke, in a non-traumatic setting. The ultrasound workup found a dissection of the abdominal aorta extended to the common iliac arteries, the ascending aorta and the common carotid arteries. No signs of aortic rupture were observed. The patient could not be operated and received a conservative medical treatment without anticoagulant or antiplatelet agent. Apart from the occurrence of left monocular blindness, the clinical course was uneventful after three weeks of hospitalization.
