Takayasu arteritis (TAK) is a large vessel vasculitis of unknown etiology, Women are affected in 80 to 90 percent of cases, with the age of onset is usually between 10 and 40, it has a worldwide distribution, with the greatest prevalence in Asia. The current case report focuses on a middle age with history of aorto-iliac thrombosis and amputation of the right leg (above knee amputation) due to ischemia. Patient complained of weakness, malaise, fatigue, left lower limb manifestations of ischemia, abdominal pain,her investigations showed acute kidney injury, creatinine 550 μmol/L and mild anemia 9 g/dL. CT angioshowed: focal narrowing of the abdominal and thoracic aorta, extensive thrombosis, renal infarction, splenic infarction. TAK is not limited to Asia but is present worldwide. Early diagnosis and treatment is warranted. Delay in diagnosis: vascular disease may start and progress to severe irreversible lesion.
