<p>Malakoplakia is a rare granulomatous benign disease. The most supported pathophysiological theory is the infectious origin associated with an impairment of macrophage function. Its clinical or paraclinical aspect does not have any particularity feature, in contrast to its histological specificity characterized by the presence of Von Hansemann's cells. The pseudo-tumoral shape and the tubo-ovarian location are rare. The treatment is mainly medical combining antibiotic and cholinergic, surgical excision is indicated only in pseudo-tumoral forms or in case of total destruction of the affected organ.</p><p>We report a case of a tubo-ovarian pseudo-tumoral malakoplakia simulating an ovarian cancer and we discuss the current knowledge on this rare disease and its therapeutic approach.</p>
