Abdominal aortic aneurysms in patients with coronary artery disease

Summary : Abdominal Aortic Aneurysm (AAA) is defined as a localized and permanent dilation of the abdominal aorta. It is a major public health problem because AAA rupture is the tenth leading cause of death in American men over 55 years old. Despite the fact that AAA and atherosclerosis share several risk factors, no specific recommendations have been given for screening patients with advanced atherosclerosis. The high prevalence of coronary artery disease (CAD) among patients with AAA is well known. Indeed, coronary investigations are often required prior to aortic surgery. In contrast, the opposite relationship has only been investigated in some recent cohorts or in subgroups of patients with limited, incomplete and often conflicting results. In this review, based on a thorough review of the literature including personal experience, the authors report the prevalence and predictive risk factors of AAA among CAD patients. They focus on the characteristics of AAA in CAD patients (definition, prevalence, diameter distribution…) and described the clinical profile of these CAD patients with AAA, including coronary profiles and associated atherosclerotic locations (carotid artery stenosis and peripheral arterial disease). They attempt to identify CAD patients at high risk for AAA development and discuss the usefulness of AAA screening in these subpopulations.
