Abnormalities of the microcirculation and hypertension: cause or consequence?

Summary : Experimental and clinical studies have established a link between target organ damage in hypertensive patients and severe defects in tissue perfusion and the microcirculatory structure and function. Microvascular rarefaction is a constant hallmark of arterial hypertension and likely contributes to higher systemic resistances and lower tissue perfusion. Alteration in arteriole reactivity resulting from endothelial dysfunction is also characteristic of the impaired microvasculature of hypertensive patients. Thus, both effectively lowering the level of blood pressure while preserving or restoring optimal tissue perfusion is currently one of the major challenges for antihypertensive treatments in order to improve the cardiovascular prognosis of hypertensive patients.
