Hypercholesterolaemia in childhood

Summary : Hypercholesterolemia is a frequent problem in childhood. Published reports demonstrate that atherosclerosis begins in childhood and that the cholesterol level in children is a risk factor for this process. The data showing the prevention of cardiovascular disease, both in primary and secondary interventions, by decreasing the cholesterol levels in adults can be extrapolated to children. Therefore the screening of children for high blood cholesterol is desirable. The modalities of the screening: of the whole population or of selected children groups remains a matter of controversy. Screening could start after four years of age, when the tracking of the cholesterol levels into adults levels starts, even if still unsatisfactory. Once identified, the hypercholesterolemic child should be treated. Dietary intervention is the first line of therapy and should be adapted to the nutritional requirements of each age. Bile acid-binding resins are the drugs of choice if pharmacologic treatment is needed. There is still no scientifically determined thres-hold for drug intervention. The importance of involving the whole family in the treatment process is emphasized.
