Adenosine and cardioprotection during acute myocardial infarction |

Summary : During myocardial infarction some patients present microvascular lesions induced by reperfusion. These microvascular lesions are responsible of a myocardial no-reflow, often associated to a worse prognosis. Experimental and clinical studies are conducted to test substances designed to reduce the incidence of reperfusion-induced no-reflow. Among these substances adenosine has the most promisising results. Adenosine has various biological effects: inhibition of platelets and neutrophils activation, inhibition of free radicals, arteriolar vasodilatation, preconditioning. In animal models treatment with adenosine before ischemia-reperfusion is associated with a smaller infarct size. In men, some preliminary results are promising and larger trials will soon be started. |
