Pathogenesis and underlying mechanisms of varicose veins: review and future prospects

Summary : The pathogenesis and the mechanisms of varicose veins are still poorly understood. Numerous risk factors have been advanced in the development of varicose condition but most epidemiologic studies were open to methodological bias. Two theories have been postulated for varicose veins: valvular incompetence and venous wall weakness. Among the possible mechanisms leading to histologic changes observed in varicose vein wall, insufficient oxygen supply to the medial layer is likely the most relevant. Venous wall hypoxia is a potent stimulus for growth factor secretion and hypertrophy of the venous wall. Fibrosis then sclerosis follow this metabolic dysfunction.
