Cardiovascular safety of sildenafil |

Summary : The cardiovascular safety of sildenafil may be assessed by the registration dossier and on the follow-up of patients using sildenafil since its introduction in the USA in March 1998. Sildenafil has systemic haemodynamic effects. Nevertheless they are small and often without clinical significance. The drug has been studied in association with antihypertensive drugs. The incidence of adverse effects is not higher than that observed in patients without antihypertensive drugs. In patients with cardiopathies, the incidence of adverse effects is comparable to that observed in patients without cardiopathy. The major adverse effects are due to vasodilation : headache, flush. The association with NO donors is contra-indicated (nitrates, molsidomin, potassium channel activators). The fall in blood pressure may be dramatic with these associations. This contra-indication is absolute. Concerning the analysis of deaths, there is no extramortality with sildenafil. The rate of myocardial infarction and death does not seem to be different to the expected rate in a comparable population. |
