Antihypertensive therapy and tobacco

Summary : Smoking is a modifiable cardiovascular risk factor. The use of this term must remind the doctor that smoking cessation is an axis for priority care in smokers with hypertension. Our work confirms the existence of an interaction, in the sense of a diminishing effect for two clinical outcomes: strokes and major cardiovascular events. This could be likened to a potential pharmacological interaction between tobacco and b-blockers described by some authors. We cannot definitively conclude on the effect of antihypertensive treatment in smokers. An excess risk to treat hypertensive smokers cannot be rejected. The results of this preliminary study require further developments with a detailed comprehensive meta-analysis but they could represent a first step towards a reassessment of the terms of management of hypertension in smokers.
