Prostate Cancer “Screening” or “early detection”?

Summary : Many countries recommend not “to organize” a systematic screening for prostate cancer and to stick to the principle of early and individual detection. The PLCO and ERSPC controlled studies confirm the absence of reduction in overall mortality, cast doubt on a benefit in terms of specific survival, with no prospects to gains and losses (in years of life or morbidity). The only approach currently being considered for early detection requires a joint decision between patient and his doctor. The highest ethical constraints on that concern the information, clear, fair and complete, on risks, benefits and particularly uncertainties of the downstream chain of diagnostic and therapeutic that may result of this approach.
