Summary : NSAIDs remain the therapeutic class of reference, consider the front line in spondylitis.
Of the traditional DMARDs, sulfasalazine alone is of interest in spondylitis, only for peripheral forms of the disease.
The anti-TNFa have demonstrated their effectiveness on various clinical manifestations of spondylitis. They should be prescribed only second line after failure or inadequate response to NSAIDs. There is no evidence that they can slow down the spine ossification and ankylosis therefore characterizes this disease.
The management of spondylitis also includes non-pharmacological measures including rehabilitation, education and therapeutic medical and social measures.
