Dementia with Lewy bodies: use of clozapine in the treatment of behavioural disorders |

Summary : Based on an autopsy series, the frequency of dementia with Lewy bodies has been evaluated to be 15 to 36% of the dementia cases, which would make it the second cause of dementia after Alzheimer's disease. It is characterized by fluctuant cognitive disorders, very precise visual hallucinations and Parkinson's syndrome which appears late after the cognitive impairment. The treatment of psychotic symptoms and behavioural disorders is rather difficult, as the patients present a neuroleptic hypersensitivity which worsens Parkinson's syndrome and can induce acute agitation. Nevertheless, these disorders are the first cause of family suffering and hospitalization or institutionalization. Yet, there are no real treatment guidelines although the choice of the antipsychotic is of the utmost importance. Here, we report the case of a patient who presented with behavioural disorders and agitation under neuroleptic treatment and who was subsequently treated by low dose clozapine (25 mg/day) which proved effective. |
