MSI colon cancer: is adjuvant therapy warranted? |

Summary : Microsatellite instability (MSI) phenotype occurs in approximately 15% of colorectal cancer (CRC) patients and may be sporadic or hereditary. Several studies have shown that patients with MSI CRC have a more favourable prognosis than those who have microsatellite stable CRC. Recent clinical data indicate that patients with hereditary MSI tumor treated with 5-fluorouracil (5-FU) as adjuvant chemotherapy may have a survival advantage, in contrast to sporadic forms that would not benefit. However, the benefit of oxaliplatin-based adjuvant regimens regarding MSI phenotype remains poorly investigated. Some preliminary data from retrospective studies suggest that addition of oxaliplatin to 5-FU could provide a survival advantage for patients with stage III MSI tumor. Nevertheless, these results should be confirmed by ongoing analysis of tumor samples from previously published randomized trials comparing 5-FU to FOLFOX (NSABP-C07 and MOSAIC studies). |
