Summary : Diffuse large B-cell lymphomas (DLBCL) constitute the most frequent type of adult lymphomas. They are heterogeneous, clinically, morphologically, phenotypically and molecularly. However, among them, some clinicopathological entities are recognized. Molecular signatures based on gene expression have identified three well-defined entities mainly based on the concept of cell of origin: GC (germinal center), ABC (activated B-cell), PMBL (primary mediastinal B-cell lymphoma), the last one sharing some gene expression profiles with classical Hodgkin lymphoma. Complex genomic alterations, sometimes more specific of one of these three entities have been defined. Major oncogenic pathways, often involving bcl-6, NF-κB, IRF-4/mum1 represent rational therapeutic targets. However, the regulations of these pathways are complex linked to the additional genomic alterations in tumor cells, the microenvironnment and host’s polymorphism. |
