Temporal lobe epilepsy surgery: a non invasive presurgical protocol (a moroccan experience) |

Summary : The Clinical Service of Neurophysiology recruits a great number of epileptic patient with drug-resistant partial seizures, who are good candidates to surgical treatment. We report our recent experience in the non invasive presurgical evaluation based on establishment of anatomical and electro-clinical (interictal and ictal) data. On a period of 2 years, 73 patients have been investigated and we selectioned 40 patients with temporal seizures related to Ammon Horn sclerosis (30) or mass lesions (10). Clinical, interictal EEGs and anatomical data were strictly correlated among 8 patients and that was confirmed by the ictal video-EEG monitoring. Moreover, it was of a great contribution for patients who presented discordance between the anatomical lesion and interictal EEGs (2 patients) or unusual EEG patterns for temporal lobe seizures (bilateral abnormalities (8 patients) or lack of EEG abnormality (5 patients). In addition, video-EEG monitoring has allowed to rectify the diagnosis for 20 patients. 14 patients have been operated with a good post-surgical outcome for 85 %.These data presented here point out the validity of our non invasive presurgical protocol in surgery of partial chronic temporal seizures. |
