Drugs that have shown their efficacy in diabetes mellitus are ACE inhibitors and statins. However, guidelines focus on intensive glycemic control although there is no proof of its clinical efficacy. How does it influence GP's prescriptions? When they are confronted with a diabetic patient who is treated with metformin who has a HbA1c of 8.1%, 84.4% of GPs prescribe a second drug of which 92% are an oral antidiabetic agent. 85.4% want to improve the HbA1c. Medical prescriptions are based mainly on a biological objective which does not correspond to Evidence Based Medicine.
