Permanent female sterilization by micro-insert (Essure ® system) placement: a feasability study Volume 23, issue 4, octobre-novembre-décembre 2004

In France, tubal ligation by laparoscopy is the reference method of permanent female sterilization. To avoid surgical incision and general anesthesia, a new method based on expanding micro-insert placements (Essure™, Conceptus ®) in the fallopian tubes by hysteroscopy is now available. This study aims to report the first 24 cases of permanent female sterilization by this new medical device in our hospital. Patients were 39.5 year-old [range: 31-47], weighted 72.0 kg [49-110], and measured 165.3 cm [155-180]. They had been pregnant 3.5 times [0-11] and had 3.2 children [0-10]. After the first attempt, bilateral microinsert placement was achieved in the 24 patients (success rate: 95.8%) without any adverse event. Hysteroscopic procedure lasted 12.7 minutes. The length of stay did not excess two days (mean 1.16 day for implantation under general anesthesia and less than one day for implantation under diazanalgesia). Literature data show a 14% failure to achieve bilateral placement of devices at the first attempt. After a successful placement, there was no report of pregnancy after a two-year follow-up. Our preliminary results are in agreement with literature data about the immediate safety and success of this procedure and have lead the hospital therapeutic committee to chose this procedure as new standard.
