A comprehensive evaluation of the drug use process was conducted at the Blois Hospital Center. The aim of this study is to identify the weakness of the system, to establish and prioritize corrective actions in order to prepare accreditation and computerization of this process.A clinical practice survey and a process analysis were performed at Blois’ Hospital by the drug committee with a strong involvement and approval by the team dedicated to the project.This assessment was made on prescription, administration and dispensation of the medication with an important communication plan.The survey totalized 237 questions, 31% of hospital beds were concerned and the rate of answer was 99.6%. Only answers not corresponding to referential with a frequency of observations of at least 30% were analysed.The most frequent malfunctions observed in prescriptions were: use of home treatment without written medical authorization, lack of legal mentions on prescriptions (weight of patient, day and hour of prescription, pharmaceutical form of drug, etc.). The malfunction in administration was prescriptions rewriting. Finally, the malfunctions in the dispensation were: no quality assurance system in pharmacy and name specific dispensation without computerized prescription and without control of the dispensation.Ways to improve the prescription and administration process were proposed with the audited persons after restitution of the results, and the professional committee for the dispensation. An action plan was written and quality indicators in order to evaluate the impact of this study are under development.
