Paraquat, a bipyridyl herbicide derivative, can lead to serious human poisoning following ingestion. We relate an acute accidental poisoning in a man with high alcohol blood level. A review of paraquat toxicity is presented concerning more particularly mechanisms of toxicity, symptomatology, therapeutic implications and prognostic factors associated with blood and urinary paraquat levels. The precocious death of the case report, associated with particular toxicokinetic data, demonstrate that acute alcohol consumption must be added to usually studied prognostic factors concerning acute paraquat poisoning and/or that alcohol is not a protection from acute paraquat poisoning as occasionally described.
