Evaluation of therapeutic education of HIV-infected patients in Argenteuil hospital Volume 28, issue 3, juillet-août-septembre 2009

In the aim of assuring a better care for HIV-infected patients, a consultation of therapeutic education is organised in Argenteuil hospital since 2006. Provided by a pharmacist, the consultation takes place weekly within the clinical department. Patients are referred to by physicians the same day or few days before the initiation or the modification of their antiretroviral therapy, or when the patient interrupts his therapy. Relevantly, we evaluated the impact of these consultations over the year 2007, with, as an objective, the continuous amelioration of this practice. CD4 cell count and plasma viral load (on day 0, 6th and 12th month, respectively) have been used as an indicator of the patient’s adherence. A questionnaire has also been conceived in order to evaluate the competences obtained by the patients during the programme. Finally, an inquiry of satisfaction’s patients has been realized. The biological reports of 33 patients have been analysed. The viral load of naïve patients is undetectable for 86% of them at the 6th month and 93% at the 12th month. For previously treated patients, the proportion of undetectable viral load is 32% at first consultation and 84% at the 6th and 12th months. Twelve patients answered the questionnaire (76% of them are sub-Saharan Africans). Their medical treatment programme, as well as the contamination and prevention means were known (11 patients). However, the modalities of infection survey (6 patients) and the extent of compliance (7 patients) were less known. All the patients seem to be satisfied with their educational consultations and eleven of them believe to become less worried about their disease. This inquiry shows the advantages of conducting a therapeutic education programme. The encountered failures are often due to the language barriers, social difficulties and psychological problems. Therefore, a pluri-disciplinary action is required for the amelioration of patient’s adherence.
