Nutritional care evaluation of patients receiving extemporaneous ternary mixture Volume 21, issue 1, Mars 2002

To assess the quality of nutrition care of patients treated by Vitrimix®, an evaluation has been performed in all clinical wards of cochin hospital for 3 months. A reference document concerning indications and quality of intravenous and enteral feeding was established from consensus conferences of 95, 97 and 98. Thirty patients receiving Vitrimix® were included at the beginning of nutrition and monitored during their treatment. The results have been compared to the consensus. We have determined the energy expenditure, the body mass index and the weight loss for each subject. The reasons of hospitalisation of patients have been cancer (n = 20), surgery (n = 4), nutritional depleted status because of HIV (n = 3) and inflammatory injuries (n = 3). The duration of intravenous nutrition by Vitrimix® were to 5 from 44 days. Two third of patients have been considered in nutritional depleted status by medical examination. The calculation of the weight loss has shown that only one third of them were nutritionally depleted. According to consensus conferences, only 13/30 subjects should have received first, a parenteral nutrition. 27% of patients have received suitable parenteral nutritional according to the indications, their energy expenditure and the duration of the treatment. In conclusion, this study has shown that nutritional needs were not well evaluated. The principal reason was the lack of a real nutritional status evaluation. Consequently, we have decided to propose a booklet with nutritional guidelines to our physicians and to follow all parenteral nutrition. Substantial savings could be realised by improving the collaboration between medical and pharmaceutical profession in hospital associated in the committee of nutrition and feeding.
