The aim of the study was to determine the clinical impact of pharmacists' recommendations on patient care and to document the types of most significant recommendations for the patient. Clinical impact of pharmacists' interventions in a medical care unit were assessed by a physician and a pharmacist. Hundred thirty-six recommendations in patient drug therapy were collected during 3 months. Thirteen per cent of them were considered to have no effect on patient's outcome, 65 % were significant, and 22 % very significant. Concerning the very significant recommendations : 47 % dealt with drug choice, 27 % were related to anticoagulant drugs and 43 % concerned drug interactions. These recommendations can reduce length of stay. Pharmacists' recommendations to the medical team could improve patient's outcome, prevent serious iatrogenic diseases, and also decrease the patient length of stay. An other study will focus on costs saving by pharmacists' recommendations.
