Objectives: Regarding the implementation of medication reconciliation in France, distinct issues have been raised. To promote this activity and to meet the healthcare professionals needs, a regional resource centre specialized in medication reconciliation was created. Methods: The resource centre is supported by the medicine, medical devices and therapeutic innovation observatory. It is held by experts in the field of medication reconciliation and clinical pharmacy, working in different health facilities, both private and public, with various healthcare activities. Missions are carried out: giving information, raising awareness of healthcare professionals, pooling of resources and work experiences in the region, and assessment of the activity of medication reconciliation. Results: Since its creation in January 2016, several tools have been provided to healthcare professionals: a slideshow to inform about medication errors, an educational program and two flowcharts to describe and explain how to achieve medication reconciliation, and indicators which assess the impact of the activity. These tools have been developed by the resource centre members, based on existing work and its adaptation. They are available online. Conclusion: The regional resource centre allows for the sharing of good practices and helping tools in the development of medication reconciliation. It provides a practical feedback of local experiences and it keeps evolving in order to assist professionals in the development of medication reconciliation in their own facility. Evolutions are expected this year through the opening of the education program and the expansion of the initial work group to primary care professionals.
