We hereby report a case of a hepatosplenic T-cell lymphoma and highlight the importance of considering this diagnosis. Our patient was receiving a long-term immunosuppressive treatment and presented cytopenias, B-symptoms, no lymphadenopathy and non-specific clinical characteristics. When cytopenias are discovered in a patient under an immunosuppressive treatment for a digestive auto-immune disease, a bone marrow aspiration and biopsy are required and a diagnosis of hepatosplenic T-cell lymphoma must be considered. Despite a large variability in morphology, the clinical presentation, the immunophenotype and the sinusoidal distribution of the cells allow the diagnosis to be established.
